The empty chair: what no-shows cost a clinic

Every missed appointment is a slot nobody else can use. The good news: one of the simplest fixes in healthcare is also one of the best researched.

Ask anyone who runs a clinic what frustrates them most, and no-shows come up quickly. The doctor is ready, the room is prepared, and the chair stays empty. That slot can't be sold again, and another patient who wanted it was told there was nothing available.

How big is the problem?

Bigger than it looks from a single day's diary. A Cochrane review, one of the most rigorous types of evidence in medicine, pooled randomised trials of appointment reminders. In the groups that got no reminder, attendance was 67.8%. In other words, roughly one in three appointments was missed.[1]

No-show rates vary a lot by specialty and country, so your own number may be lower. But even a small percentage, across every day of the year, adds up to a lot of lost time and revenue.

The fix is simple and well proven

The same review found that text message reminders raised attendance from 67.8% to 78.6%. Phone call reminders worked about as well as texts, with attendance of 80.3% in the studies that compared them directly.[1]

The catch with phone reminders has always been staff time. Calling every patient the day before is a big job, so most clinics fall back on texts alone. And texts are one-way: a patient who can't make it often doesn't bother replying, so the slot still goes empty.

A reminder that lets the patient reschedule on the spot doesn't just prevent a no-show. It gives the slot to someone else.

Where a voice agent helps

An AI voice agent can make reminder calls at scale and, more importantly, have a real conversation. If the patient needs to move their appointment, it can offer new times and rebook them during the same call. The freed-up slot goes straight back into the diary for the next patient who calls.

If you run a clinic, start by working out your no-show rate for the last three months and what each missed slot is worth. That gives you a clear target, and a clear way to tell whether any change, AI or not, is paying off.
